But there — tucked away nicely in the eleventh paragraph, without subheading or italics or boldface or sidebar ("too damn bad we don't have footnotes we could drop this into," you can almost hear the editors murmur) — we finally learn the key fact to answer the questions, "How could this have happened and who's to blame?"
