Question about Steve Irwin

From the news:

Fatally injured "Crocodile Hunter" Steve Irwin pulled a stingray's serrated barb from his chest before he lost consciousness and died, his manager said on Tuesday as fans worldwide mourned the exuberant naturalist.
I believe it is standard procedure to NOT remove a foreign object from one's body if one has been impaled by said object, yes? The reasoning being that an object that has invaded your body and remained inside your body is likely plugging any holes that it has made in blood vessels; you leave it there to act as a kind of stopper until you can get to hospital or some similar medical facility.

I am certainly not second guessing the guy, who was doubtlessly in shock and in considerable pain, but I wonder if he was fatally wounded upon being impaled or if he sealed his fate by removing the barb.

Or both.
